Norman Fogle, M.D.
Dr. Norman Fogle grew up and continues to make his home in Indianapolis, IN. He stayed in his native state for his pre-med and medical education, attending Indiana University and the Indiana University School of Medicine. Training in his chosen specialty was completed in Community Hospital’s Family Practice Residency program. He is a member of the Indiana State Medical Association, the American Medical Association, and the American Academy of Family Physicians, and is board certified in family medicine.
Outside of his medical practice, Dr. Fogle devotes his time to the Jewish Community Center Board, and enjoys travel and the outdoors. His professional interests focus on heart health, early detection of disease, and promoting healthy family lifestyles. He and his wife, Adrienne, have three children, all launching successful careers based in the Indianapolis area.

James Kluzinski, M.D.
Dr. James Kluzinski was born and raised in Chicago, IL. He attended Triton College in River Grove, IL. He continued his education at the Jagiellonian University in Krakow, Poland and studied medicine at the Medical Academy of Wroclaw, Poland, graduating in 1986. After graduation, Dr. Kluzinski attended Union Hospital’s Family Practice residency program in Terre Haute, IN. He is a member of the American Academy of Family Physicians and the American Society of Bariatric Physicians, and is board certified in family medicine. He joined Northeast Family Physicians in 1991.
Dr. Kluzinski is very interested in preventive medicine and aggressive management of chronic diseases. He loves the old saying “An ounce of prevention is worth a pound of cure.” Dr. Kluzinski understands how an occasional simple intervention can increase a person’s quality of life. He believes chronic diseases such as high blood pressure, high cholesterol and diabetes should be managed very aggressively. Dr. Kluzinski has the ability to help people medically manage their weight and can design a comprehensive customized weight management program for his patients.
Dr. Kluzinski is a good fit for patients who want a caring physician who will spend time explaining options in simple, straightforward fashion. He is a great choice for those who want to be actively involved in their care and personal health, and who believe that prescription medications are NOT always the answer.
Dr. Kluzinski’s leisure time is spent with his family and pets. He enjoys outdoor activities that include bicycling, canoeing and hiking.
Tammy Polit, D.O.
Dr. Polit welcomes patients of all ages from newborns through adults. She emphasizes the complete care of each patient as an individual and actively works with them to pursue the best health that they can achieve. Dr. Polit has been in private practice for over ten years, after serving as clinical faculty at two Indianapolis family practice residency programs.
Originally from Massachusetts, Dr. Polit received her Bachelor of Science with a double major in zoology and postgraduate level French from the University of Massachusetts at Amherst in 1989. She went on directly to the University of New England College of Osteopathic Medicine (UNECOM). UNECOM is the only medical school in the state of Maine and the only osteopathic medical school in New England. There, she completed her Doctorate in Osteopathic Medicine in 1993. Her clinical rotations included training in surgery in Allentown PA; obstetrics and gynecology at Hyde Park in downtown Chicago, IL; internal medicine in Olympia Fields, IL; and osteopathic medicine in San Jose, CA.
Moving to Indianapolis in 1993 with her husband, Dr. Polit entered a three-year Family Practice Residency program at Community Hospital and served as the president of the Executive Council of the Indiana Academy of Family Practice Residents. Community Hospital’s residency program is the largest residency program in Indianapolis, allowing residents to pursue intensive training in all areas of medicine including pediatrics, neonatal intensive care, prenatal care, obstetrical care and deliveries, gynecology, emergency care, intensive care unit Interventions and procedures, general and subspecialty surgical training, oncology, and internal medicine as well as other aspects of medicine in a large urban hospital setting. Dr. Polit’s residency training also included focused clinical rotations at several prominent institutions in the San Francisco area. These included hematology/oncology, internal Medicine, and nephrology at San Jose Medical Center, San Jose, California and Advanced Osteopathic Manipulative Therapy, San Jose, California.
Dr. Polit has also been active internationally, participating, presenting and authoring medical publications at a number of European medical conferences beginning in 1995. In 1999/2000 Dr. Polit spent two semesters at the Universite Paris Sorbonne in Paris, France where she enjoyed the opportunity to interact with European colleagues.
Board certified in family medicine, Dr. Polit is a Diplomate of the American Board of Family Medicine, a member of the American Academy of Family Practice, and a member of the Indiana Academy of Family Physicians. She is on active staff at Community Hospitals of Indianapolis/ Community Health Network. Dr. Polit is an active member of Community Physician Network and has served several years on the Quality Care Committee and on the Business Excellence Committee.
Dr. Polit’s specialties include newborn physicals, well child care and immunizations, and adolescent care; gynecological care such as pap smears, diagnostic colposcopy, therapeutic cryotherapy; and comprehensive care of all adults.
Her special interests include dermatology, Accutane therapy, Botox facial and axillary therapies, cosmetic filler therapies, office surgeries such as skin lesion biopsies, laceration repair, growth and wart removal; vasectomies that include traditional, VasClip, and no scalpel techniques; advanced cholesterol testing and treatment using NMR testing by Liposcience, and osteopathic manipulative therapies, including craniosacral technique.
Dr. Polit and her husband have two children. In their spare time, they enjoy wakeboarding, snowboarding and traveling.

Asoka Ratnayake, M.D.
Dr. Ratnayake was born and raised in Valparaiso, Indiana. He graduated from Lafayette College with a degree in electrical and computer engineering and earned his medical degree from Indiana University School of Medicine. He completed a family medicine residency at St. Francis Hospital and Health Center before joining Northeast Family Physicians in September 2010.
As a board-certified family physician, Dr. Ratnayake enjoys treating patients of all ages, from pediatrics to geriatrics. He looks forward to creating lasting patient-physician partnerships focused on healthy living. Dr. Ratnayake is pleased to be part of the dedicated group at Northeast Family Physicians and is accepting new patients.
Dr. Ratnayake and his wife live on the north side of Indianapolis. His favorite leisure time activities are playing the piano and swimming.
John Tzucker, M.D.
Dr. John Tzucker founded the group that is now Northeast Family Physicians in 1977 with his partner, Dr. Fogle. He is a native Hoosier. He attended Indianapolis’s North Central High School, Indiana University for his undergraduate degree and The Indiana University School of Medicine for his medical degree. Dr. Tzucker’s family practice residency was done here at Community Hospital. He is board certified in family medicine and a member of the Indiana State Medical Association.
Dr. Tzucker reserves a lot of his leisure time for physical fitness activities, and especially loves to combine this with support of his favorite local charity, Riley Hospital for Children. Thus, in addition to the Indianapolis 500 Festival Mini Marathon, he would hate to miss the annual “Bop to the Top” of the OneAmerica Tower. Special professional interests include sports medicine, pediatrics and management of chronic disease. Dr. Tzucker and wife Joan make their home in Carmel, Indiana. They have raised four children, now scattered to the four corners of the country.
